A 14 yr old girl came to opd with CHIEF COMPLAINTS : Shortness of breath since 2 days 
Vomitings since 2 days
Fever since 2 days
History of presenting illness:
Patient was apparently asymptomatic 2days ago thn she developed Shortness Of Breath which was insidious in onset, gradually progressive from grade 1 to grade 4,not associated with orthopnea,PND
FEVER which was insidious in onset, high grade ,continuous,no diurnal variation associated with chills n rigor
VOMITINGS which was

65 yr old patient with pain in lower abdomen and Shortness of breath

A 65-year-old male patient  who is a farmer once(stopped working 6 years back)came to the medical OPD with the  Date of history taking:- 5/12/22 Date of examination:- 5/12/22 CHIEF COMPLAINTS:-  Pain in the lower abdomen since 8 days  Shortness of breath since 8days HISTORY OF PRESENTING ILLNESS:-  The patient was apparently asyptomatic 8days back then he  experienced diffuse pain all over the abdomen which was insidious in onset and was not radiating and relieved on temporary medication ( drug - unknown; dose unknown; indication - unknown ) character of pain (?) A50 year old male farmer by occupation came with  CHIEF COMPLAINTS: ABDOMINAL DISTENSION Since5 Days SHORTNESS OF BREATH Since 5 Days B/L PEDAL EDEMA Since 5 days HOPI: Patient was apparently asymptomatic 5 days ago. An asha worker during kallavelugu programme noticed and advised him to go to hospital for yellowish discoloration of eyes 5months ago.So he went to local clinic and advised to use unknown medications for 5 days ,which he used it  for 3 days but it was not reduced. Then he went to hospital at khammam and the unknown investigations were done along with endoscopy, medications were given which he used for 5 to 6 days He used herbal medicine (aaku pasaru) for 3 days/ once a week/for 3 weeks Later he came here with complaints of yellowish discoloration of eyes and mild abdominal distension investigations were done 4 months ago n medications given which he used for 3 months All of a sudden the patient developed abdominal pain which was sudden in onset, gradually progress...
